辦理證件 Mainland Travel Permit Application

預約/查找/更改/取消預約 Booking/Query/Modify/Cancel Appointment

請備齊以下三類文件方可進行正式預約受理服務中心和時間。
Please ensure that you have prepared the Following three categories of documents, then you can proceed with the formal appointment to choose the service center and time slot.

	有效的香港永久性居民身份證。（未滿11周歲遞交載明其具有香港永久性居民身份的《〈索取資料申請〉通知書》。 Valid Hong Kong Permanent Identity Card. If you are under 11 years old, please submit the 《Notice of Application for Access to Information》which states that you have the status of a Hong Kong permanent resident.
	有效的普通護照（中國政府不承認的證件除外），證件有效期不少於6個月； Valid ordinary passport with a validity period of at least 6 months, excluding documents not recognized by the Chinese government.
	香港入境事務處簽發的包含國籍信息的《〈索取資料申請〉通知書》，文件簽發時間應當在申請日之前6個月內。《Notice of Application for Access to Information 》issued by the Hong Kong Immigration Department containing nationality information. This document must have been issued within 6 months prior to your application.

可供預約今天起計28天內至預定證件服務中心辦理。
You can make an appointment to apply for the "Mainland Travel Permit for Hong Kong and Macao Permanent Residents (Non-Chinese Citizens)" at our Service Centers within 28 days starting from today.
